All forms are form fillable and must be printed, signed and returned to C&C Helping Hands. Access to the Parent Handbook will be given at the interview should you choose to enroll in our child care. Each form indicates if it is required for enrollment, for informational purposes only, or only needs to be filled out if it applies to your child. Please work with Carolyn to ensure you need to fill out any optional paperwork.

All forms must be updated annually.
